At Stony Brook Southampton Hospital, we may be able to help you find health insurance options through NY State of Health. Now through January 31, 2021 we are taking appointments over the phone to enroll in a plan. To schedule an appointment, please contact us at 631-726-8926 or email us.

You will need to provide:

Federally Issued Identification, Residency Card, or Work Permit

Social Security Card

Four (4) most recent paystubs

Most recent tax return

Must be under 65 years old or not receiving Medicare Benefits.
