Southampton, NY—Southampton, NY—Southampton Hospital’s Regional Dialysis Center has been recognized for its exemplary patient safety program at its Hampton Bays Atrium site. The Five-Diamond Award is presented by IPRO’s End Stage Renal Disease (ESRD) Network of New York, which is dedicated to assisting dialysis and renal transplantation centers in establishing and maintaining high standards of care for ESRD patients. The ESRD Network’s interactive program is designed so that each dialysis facility completes one module at a time and submits documentation for review. The quality of each module determines the number of “diamonds” awarded, with a maximum of 5 diamonds, within the participation period. Only 11 facilities out of 243 in the State have currently attained this level of patient safety.
The Hospital’s state-of-the-art Regional Dialysis Center offers a full range of treatments to serve people and visitors throughout the East End. To meet the demand of a growing patient population, the Center expanded hours to include evening appointments for those who are working during the day. Gaylord Hoffert, MD, Medical Director of the Dialysis Center, Board-certified in Nephrology and Internal Medicine. Robert J. Weiss, DO, a physician Board-certified in Nephrology and Internal Medicine, recently joined the Center. Both physicians are available for appointments at the Meeting House Lane Medical Practice in Southampton and will soon be seeing patients in a Hampton Bays location as well. Licensed by the New York Department of Health, the center is staffed by an interdisciplinary team of skilled RNs, a Certified Social Worker, and a NYS Registered Dietician.
