Cardiac Rehabilitation is a nationally certified program focused on your recovery. 

We are proud to be one of the few nationally certified Phase II Cardiac Rehabilitation programs on Long Island. Phase II is the monitored, medically supervised program that is typically covered by insurance after a heart-related event or procedure. Our team includes nurse practitioners, cardiac nurses, exercise physiologists, and physical and respiratory therapists — all working together to support your recovery and long-term heart health. Many patients feel so supported here that they choose to continue exercising with us long after their rehab program ends.

 

Phase I- occurs before your discharged from the hospital. 

Phase II- soon after your event, offered at the hospital in an outpatient setting, typically covered by insurance 

Phase III-Maintenance exercise program

Who Qualifies 

You may be eligible if you have had: 

  • A heart attack (within 1 yr)
  • Angina 
  • Coronary bypass surgery or valve surgery 
  • Angioplasty or stent placement
  • Heart failure

Cardiac Ongoing Exercise Program 

Maintenance Program: A self-pay option for people who want a safe, supportive place to exercise — whether you’ve completed our monitored Cardiac Rehab Phase II or you simply want guidance from our trained cardiopulmonary team we are here to support you. 

This program offers: 

  • Access to the same gym and equipment used during cardiac rehab 
  • Staff present at all times in case you have symptoms or concerns 
  • Continued guidance when you need it 
  • A strong sense of community and connection 

This program is ideal for anyone who wants to stay active with the reassurance of trained staff nearby, even if they never participated in the monitored, insurance-covered phase of cardiac rehab.
